Introduction – Appearance (Cleanliness, Safety, etc.): On a scale of 1 to 5, rate your impression of the business's appearance upon arrival.

(Poor): The premises were unclean, disorganized, and lacked safety measures.
(Below Average): Some aspects of cleanliness and safety were lacking.
(Average): The premises were moderately clean and safe.
(Good): The business maintained a clean and safe environment.
(Excellent): The premises were immaculate, inviting, and adhered to all safety standards.

Need/Want – Objective: Please rate how well the business understood and fulfilled your needs or desires.

(Poor): The business did not address my needs effectively.
(Below Average): Some effort was made to fulfill my requirements.
(Average): My needs were partially met but lacked personalization.
(Good): The business addressed my needs satisfactorily.
(Excellent): My needs were fully understood and met with personalized solutions.

Exit (Repeat Customer or Not?): Indicate your likelihood of becoming a repeat customer based on your current experience.

(Unlikely): I am unlikely to return for future purchases or services.
(Not Very Likely): The experience was average; I might consider returning.
(Neutral): My decision to become a repeat customer is uncertain.
(Likely): I am likely to return for future purchases or services.
(Very Likely): I am highly likely to become a loyal repeat customer.

Timeliness: Rate the business's efficiency in handling your requests, orders, or appointments.

(Unsatisfactory): The service was extremely slow and delayed.
(Below Expectations): There were some delays in addressing my requests.
(Satisfactory): The business provided service within an acceptable timeframe
(Timely): The service was reasonably prompt and efficient.
(Very Timely): The business responded quickly and efficiently to my requests.

Quality of Product or Service Provided: Evaluate the overall quality of the product or service you received.

(Very Poor): The product or service provided was of extremely low quality.
(Poor): The product or service did not meet my expectations.
(Fair): The quality of the product or service was satisfactory but not exceptional.
(Good): The product or service met my expectations and was of good quality.
(Excellent): The product or service provided was of exceptional quality.
